| Version | NICHQ Vanderbilt; parent/teacher; initial/follow-up forms |
| Population | Children/adolescents; verify age range for selected form |
| Respondent | Parent and teacher forms |
| Purpose | Vanderbilt ADHD rating scales — administration and scoring reference. |
Administration
Select parent or teacher and initial or follow-up form. Review symptom ratings alongside the performance section and obtain information across settings.
Scoring method
Separate respondent forms, symptom counts, impairment and comorbidity screens; not total-only diagnosis.
Interpretation & limitations
- Use the selected form’s symptom-count, impairment and additional-screen instructions. A raw sum alone is insufficient.
- Interpret the findings alongside the clinical interview, functioning, history and relevant physical assessment.
- Do not apply the initial form’s comorbidity keys to a follow-up form or equate disagreement between respondents with invalid reporting.
- Use the original instrument and manual for item wording, rating anchors, missing-data rules and any normative conversions. This reference does not establish a diagnosis or select treatment.
